| Payment of registration fees
acknowledges that you, the parent or legal guardian of the participant,
agree with the Pleasant Grove Youth Association (hereinafter referred
to as “Association”) and give your permission for your child to
participate. You certify that all information blanks contained herein
concerning your child have been filled in and are true and correct. You realize that the Association is a charitable, non-profit and voluntary program instituted for the benefit of the children of this community. You hereby agree not to hold the Association, its board members, coaches or staff responsible for any injury or damage to your child, and agree to indemnify the Association, its members, coaches and staff against, and will save each and all of them harmless, from any and all causes of action, claims, expenses and damages, including but not limited to, interest, court costs and attorney’s fees arising from injury to your child. If your child is injured playing any sport sponsored by the Association or in connection with any activity of the Association, you hereby authorize the coach or manager of his or her team as your representative to obtain such medical attention as he or she may deem necessary, including but not limited to surgery in case of an emergency, and you agree to pay all such medical and hospital charges for your child’s treatment. In the event the family physician designated above cannot be contacted, treatment may be rendered at a clinic or hospital that has been designated by the Association. |
